Medical Records are private information released only at your request. You may need to have our office send
records to another treating physician or you may need your records for your own personal reasons.
Our staff will help you with sending and releasing your medical information specifically as you instruct.
To release Medical Records you
must first sign a written release form with your birth date, full name and Social Security number at the time
of treatment to authorize release of this information.If you require your Medical Records for your
personal use there is a $25.00 fee due at the time of signature of request form. Allow 5 working days for the
records to be available. $0.25 for each page copied due at the time of pickup.
