Personal Injury Patient Information
What is a Personal Injury?
Personal injury refers to the area of medicine and law that seeks to protect victims who are injured or harmed by the action or inaction of another person or entity. A personal injury claim can be filed for injury incurred by an individual either physically or mentally, and can include damage to property.

If you are involved in a car accident and are injured you may have a personal injury claim.

To register as a Personal Injury Patient at NOMC you must provide complete information regarding your claim. If you have medical payments clause in your auto insurance coverage you will be asked to provide your policy information and numbers, your claim number and adjusters name and contact informtion. At that time you will be asked to sign an Assignment of Benifits so that this office can bill your insurance directly.

If you do not have Med Pay on your car insurance, your case may still be eligible to be accepted on a lien basis provided you are represented by an attorney who represents you in your case against the opposite party, and that attorney can verify that there is accepted liability for your claim.

A "Slip and Fall " injury is a personal injury also. This can occur when a person unexpectedly encounters a slippery, wet or uneven surface and slips and falls on the property of another due to preventable, unsafe conditions.

If you have suffered a slip and fall injury this office will accept your case on a lien basis if you are represented by an attorney only. Your attorney will need to sign a lien to promise to pay the cost of your treatment.
