Injured Workers Information

Workers' compensation refers to the money or benefits provided by an employer to an employee who is injured on the job.
The money is used to defray medical costs and recompenense lost wages. Workers' Compensation benefits are awarded regardless
fault or negligence.
The goal of workers' compensation is to return the injured employee to work as soon as possible,
while preventing the employer from unnecessary hardship or loss of business. It is a win, win system that
helps you as an injured worker to get well and return to work. And helps your boss afford to continue to employ
you and your fellow workers.

If you were injured on the job please visit The State of California Division of Workers Compensation
Information and Assistance Page or the
California Directory of Worker's Compensation Resources. If you have not already done so you can fill out the Workers Compensation
Form by clicking on the link to the left. If you have selected Dr. Thompson as your treating physician, you
will be asked to provide your workers compensation insurance information, claim number and contact information
for your adjuster on your claim. If you do not already have this information your employer is required to
supply this information to you and to your physician. Be sure to notify your employer of your injury and make
sure you both complete a form 5020 for your workers compensation insurance company. This will allow your claim
to be opened and your treatment to begin without any obstacles to your care.
