Billing Information

Medicare
If you are covered by Medicare, we will submit your claims to Medicare on your behalf. After Medicare makes
its payment, we will bill your supplemental insurance carrier for the remaining balance. If you do not have
supplemental insurance, you will be responsible for the balance.
Medi-Cal
Medi-Cal is California's Medicaid program, a medical assistance program for low-income residents funded by
the state and federal government. If you are covered under this program, please provide an eligibility card
or other proof of eligibility for each month of service.
Self-Pay
If you do not have insurance or if you are seeking care that's not covered by your insurance plan, you are
considered a self-pay patient. Payment is required in full at time of service.
Injured Workers
If you are an injured worker and are being seen for your work related injury, you or your employer must provide
your workers compensation insurance information at the time of your first visit-including the adjusters name and contact numbers, as well as,
the accepted injury/claim numbers.
Personal Injury/Lien Patients
We accept personal injury lien cases. If you have been referred by an attorney for injuries suffered in a auto accident or personal injury of another
nature, you must complete the forms provided at reception at the time of registration, including the authorization
by you for our office to place a lien on your case settlement. This will allow you to be treated and then the
responsible party to be billed at the conclusion of your treatment. If you do not wish to handle your personal
injury treatment payment in this fashion you will be required to become a "Self-Pay" patient type. If you have Auto Insurance
with Medical Payment coverage, we are able to bill your insurance directly, if you provide the insurance carrier
and claim number information regarding your accident/injury.
QME/SSI Evaluation Patients
If you have an appointment for a QME evaulation with the doctor you must be prompt for the lenghty appointment
which will be scheduled on a week day at 10:30 or 11:00am. Your appointment will be paid for by your workers compensation
insurance. If you fail to appear for your QME appointment your insurance will be billed for a missed appointment
and this will eventually be taken out of your settlement, so it is in your best interest to be sure to be present for
all of your workers compensation medical appointments.If you are appointed for a SSI disability evaluation,
payment is expected at the time of the appointment and you may contact Eric on our staff for further information
regarding this type of appointment. Please note we do not accept cheques.
Payment
At time of service we require payment in full of the cost of services rendered. You may pay by way of either
cash or credit card. We accept all major Credit Cards but DO NOT accept cheques.
